Please fill out the below form to report damage to your home or another area. If you don't know the exact address please put something close by so we know where to go.

We are allowing up to 10 images to be uploaded, if you have more please send them to Mpowers@madeirabeachfl.gov with the Subject of: Hurricane Ian Damage, [insert your address]

Please be patient as crews go through the city looking at all the damage.

This information will be shared with Pinellas County and FEMA for data.
